Egg Cycling & Storage

Q. I still want to freeze my eggs. Where do I go?

If you plan to start your egg/embryo freezing treatment on or before 11/13/2019, you will proceed as discussed at Trellis at 7 West 18th Street.

If you plan to start your egg/embryo freezing cycle on or after 11/14/2019, you will proceed with Dr. Oehninger at Sher Fertility Institute, located at 425 5th Avenue (entrance on 38th Street, 3rd Floor).

We kindly ask that you call us before November 13th at (844) 202-8150 to discuss the above options and confirm your preferred path forward.

Q. What happens to my eggs now that they are being stored?

For clients who have completed their egg freezing cycle, your eggs/embryos will continue to be stored at Sher Fertility Institute in NYC. They can be reached at (646) 792-7476.

Q. What happens when I want to use my eggs?

When you decide you want to use your eggs, please contact Sher Fertility Institute at (646) 792-7476. When you are ready to thaw, they can set you up with a doctor at Sher Fertility Institute and make a plan with you to time the thawing of eggs to meet your goals (fertilization, transfer etc). Alternatively, they can arrange to transfer the frozen eggs to a fertility facility of your choice.

Medical Records

Q. Where are my medical records being stored?

IntegraMed Fertility will maintain and store medical records, and will have access to them through Sher Fertility in NYC. You can also continue to access your Trellis Patient Portal up until 12/31/2020.

Q. If a patient needs records after the clinic is closed, how can they request the records?


Complete and sign the attached Medical Records Release Form, giving us formal permission to send your medical records to you. Please also return this completed form as soon as possible to TrellisHealthInfo@integramed.com.

Billing

Q. What will I be charged for treatment if I move forward at Sher Fertility Institute?


Sher Fertility Institute will honor all valid financial estimates from Trellis. If you have received a financial estimate in the past, please remember to accept it in your patient portal so that the pricing can remain valid for 6 months. We would encourage you to print a copy for your records to present at your appointment at Sher Fertility Institute.

Q. Will there be a change to my cryo-storage fees?

Your cryo-storage fees will be locked in at $600 a year for your first 24 months.

Q. I have a question on billing for insurance purposes. Who can I contact?

Q. I want to be reimbursed for my deposit. What do I do?

Please submit your request in writing to TrellisHealthInfo@integramed.com before November 13th, 2019.

Q. There was a problem with my billing and I don’t know who to reach out to.
